Ortho Hors D'oeuvres (Prompt)

Your patient is a 25 year-old right-hand dominant male who presents with right hand and arm pain. He was involved in a fist fight 2 days ago and since then he has had progressive swelling and redness of the hand, now with streaking down the arm. You tell the patient that you plan on admitting him to the hospital for IV antibiotics, hand elevation, and serial examinations, and he says, "Hand elevation? How am I supposed to keep my hand elevated all night long? I have to sleep!"
